As the parent or legal guardian of the above-named child, I acknowledges and understand that the child´s participation in Seattle Sounders Soccer Camps programs is conditioned upon my acceptance of the terms and conditions of this Waiver of Liability, Authorization and Release. I understand that soccer is a demanding physical contact sport, which could result in physical injuries to participants. I certify that my child is in excellent physical health, and I am not aware of any physical limits to his/her participation in soccer. I consent to my child´s participation in Seattle Sounders Soccer Camps and assume all risks incidental to his/her participation in soccer. I hereby authorize Seattle Sounders Soccer Camps and its coaches and administrators to seek and authorize emergency medical treatment for my child when deemed immediately necessary or advisable and I cannot be reached. In consideration of my child´s acceptance as a participant in Seattle Sounders Soccer Camps, I hereby waive his/her personality rights under RCW63.60.010 and grant to Seattle Sounders, LLC and its affiliates a license to use my child´s name, voice, signature, photograph or likeness for commercial, promotional, endorsement and publicity purposes without further consideration. On behalf of myself and my marital community (if any), I hereby release and discharge Seattle Sounders LLC and its, members, employees, contractors, organizers, sponsors, agents and affiliated entities from any and all liability, claims, demands, damages and causes of action for personal injury, property damage, and/or other loss suffered by my child in connection with his/her participation in Seattle Sounders Soccer Camps.
